Registering with us

The practice provides general medical services to patients in G31, G32, G40 .2, G40.3 and G40.4 post code areas. If you live within the practice catchment area and you are not already registered with a local GP, you are welcome to register with us.

Please note we offer an on the day appointment system . For more information, please read the appointment system.

OUR PRACTICE POLICY IS NOT TO PRESCRIBE: Benzodiazepines including diazepam and sleeping tablets.  Anyone wishing to join the practice will be titrated off these medications.

To register with the practice you will need to complete a registration form which can be downloaded here.

Step 1 -Download the forms

To save the completed form, choose File>Save as and rename the file. You may need to download adobe acrobat reader to edit and save your form.

Step 2 – Attend surgery with completed forms and identification

Please attend the practice with the completed forms and two forms of your identity.
One form of identity must contain a photo of yourself and the other must provide evidence of your current address.

Patients are advised to read the NHS counter fraud declaration before signing the registration form.

There are a number of reasons why you may not be able to register with the practice For example, you may live too far away. If this is the case simply choose another GP in your local area.

Acceptance on to the list is as at the discretion of the Doctor.

If patients move outside our catchment area we will ask them to register elsewhere. We understand this is sometimes upsetting, especially to patients who have been with us a long time but we do have to set limits to our practice area.

Please note NHS Medical Cards are no longer supplied by Greater Glasgow and Clyde Health Board.
